Easy Ordering: Send order via Parachute Health (recommended), DMEscripts, EMR
order entry or traditional fax.

Welcome Call: Prior to shipping any equipment, our team of client success experts meets with your patient to ensure they’re educated on the order process and are confident in the next steps.

Step 3
Order Processes and Ships: The equipment typically arrives to the patients’ address within 1-2 weeks (assuming no delays with proper paperwork or insurance).

Step 4
Automated Re-Supply Program: Our team handles all re-supply needs and proactively communicates with the patient in advance of them running low on their supplies.
How to Order
Ordering your patient’s equipment and supplies through Specialty Medical Equipment requires the following documentation:
1. Prescription/Order Form: Prescriptions for durable medical equipment can be submitted through e-prescribing platforms Parachute Health (recommended) and/or DMEscripts, EMR order entry, or traditional fax.
Your SME account representative will provide you with SME order forms for traditional fax. All orders must be signed by the treating clinician and sent along with the following documents.
2. Patient Face Sheet including patient demographics, contact information, and health insurance information.
3. Patient Chart Notes including documentation supporting the medical necessity for the coverage of the equipment/supplies. Chart Notes should be from within the past (6) months from the order date.
